Documents you need

The following information is provided as a general guide. The application form itemises the list of documents required by us for your skills assessment.

Original documents

You will need to provide 600dpi colour scans of the original documents to us for assessment.

  • Original documents are documents that you receive directly from an organisation, university, or registering authority. The issuer provides these documents to you directly and they are to be provided to us in their original format. eg. degree transcript or registration letter. 

Types of Required Documents

Below is a basic list of the types of documents you will be asked to provide.  Your application form has the exact documents you will be required to provide.

PLEASE NOTE: You will need to provide clear and complete colour scans of your original documents scanned at 600dpi resolution or higher. Assessment officers must be able to see the complete document, including all edges and corners, any images/photographs and able to read all text clearly.

Personal identification

You will be required to provide us with:

  • Your original current, valid passport
  • A passport-sized photograph of yourself taken within the last 6 months.

 

Academic qualifications

You will be required to provide us with evidence of all university academic qualifications you hold including:

  • Your original degree certificate (or letter of completion if a certificate has not yet been issued)
  • Your original academic transcript

 

Evidence of registration

You will be required to provide us with:

  • Your original initial registration in Australia (if applicable)
  • Your original letter confirming current professional status (letter of good standing) in Australia (if applicable)
  • Your original initial registration overseas (if applicable)
  • Your original letter confirming current professional status (letter of good standing) overseas (if applicable)

 

Work Experience

You will be required to provide us with:

  • Your Curriculum Vitae
  • Original work reference templates (You need to provide these only if you have been working as a fully registered pharmacist and wish us to comment on your work experience)

 

Name usage on supporting documents

Please note the APC must be satisfied that all documentation provided by an applicant with an assessment application relates to the same person. Applicant name/s on the supporting documents must match with the applicant’s name on the identification document (eg. passport).

If there is name change, evidence of name change (eg. marriage certificate) must be provided by the applicant.

Translation

Documents in a language other than English will need to be translated. The APC will only accept translations completed by a translator accredited by the National Accreditation Authority for Translators and Interpreters (NAATI). The original English translation must accompany the document issued in the original language. You must scan both documents (scan of original language document and the English translation) together in one scan.
