Document Checklist

There will be various documents that you will need to provide at each stage of the process. These documents provide evidence of your identity, education background, work experience, skills and background checks.

This page is for pharmacists who received their qualifications outside of Australia or New Zealand. It is intended for general guidance only and should not replace your own research, judgement, or seeking independent advice.

Checklist

Below is a checklist of documents you may need to provide to various organisations. Depending on your personal circumstances, this list may not be exhaustive. There may be other documents you will need to provide that are not in this list.

Any missing or untranslated documents can slow down the process. It is important that you gather all of your documents early and make sure each document is in English or translated by at NAATI certified translator.

Please note that the list above should be used as a guide only. It is best to seek advice from the organisation you will be submitting documents too.

APC Document Tips

• Check that you have all the correct documents and information ready before you submit an application/visa EOI etc.
• Have all non-English documents translated by a NAATI certified translator.
• Check your name is spelt correctly across all your documents. Your names spelling must match the spelling on your primary ID document e.g. your passport.
• Keep an eye on your inbox. If an organisation needs further information from you or an amended document, they may email you.
• Ensure you provide a clear image of the original document - some organisations will not accept images of a copy, even if you get it certified.

Document FAQ's

Why may I need to provide additional documents?

Additional documents may be requested when there is an issue with the document you have submitted. For example, the spelling of your name in your academic transcript may not match your passport. If this is the case, you would need to provide evidence of your name being changed. You can do this by supplying a document such as a marriage certificate or a same and one letter to account for this discrepancy. This requires you to re-upload your document again.

Why was my document rejected?

There are a number of different reasons your document might not be accepted. Some reasons might include:

• the document is an unclear photocopy
• the document is a certified copy, not a copy of the original document
• the document is not translated by a NAATI certified translator
• the document is suspected of being fraudulent

I cannot get one of the required documents, what can I do?

If you do not have one of the documents required, you may need to contact the relevant organisation to see if there is another option or another document you can provide.

I am waiting for a document from my home country's government, will I have to wait to submit my application?

Start gathering your documents early, so you do not encounter this issue. It is best to wait until you have all of your documents before you start an application.
