This form is to be used by Supervisors for the assessment of and feedback on the intern’s performance in relation to the development and delivery of a health promotion activity. It should be used to structure the discussion and debrief following completion of the activity.
The intern and supervisor should discuss the report and evaluation report, in addition to any comments that the supervisor might have about the performance of the intern and the implementation of the activity more generally.
